Circle Family Care $52,400.00 ASSOCIATION HOUSE OF CHICAGO Family Centered Services

Association House has currently identified Ames Middle School to focus its services and design a program.

By establising collaborative relationships with parents and a specific school the program can concetrate on key truancy issues and successful interventions.

AH will partner with the school to focus on 6th -8th graders in a program of early intervention.

Target community is Humboldt Park community area.

CIRCLE FAMILY CARE Family Support/Family Prevention Circle Family Care (CFC) will provide on site school based counseling services at identified grammar schools.

CFC has established collaborative working relationships with several schools in regards to on site and/or referral services.

 Tutorial, counseling and therapy and social learning opportunities will be provided to program participants.

Two full-time staff persons will be placed on site to provide face to face intervention services.

Family therapy may be provided in the agency's office or in the home.

 Collaboration with other agencies will take place to provide recreational services, case management as appropriate and substance abuse treatment.

YOUTH OUTREACH SERVICES Academic Support  Youth Outreach Services proposes to provide case management and referrals to other agencies.

Target population is 5th through 8th grade.

 The services will target the Austin community area especially 3 identified schools.

FAMILY FOCUS, INC. Focus on Education Family Focus Lawndale proposes to primarily address Family Factors and student variables through the following education support services -- Academic Support--Monitoring of Progress--Advocacy and Positive Relationships--Counseling--Parent Component. Program will conduct outreach to schools identified in Program Plan.

Target community is the North Lawndale community area.
