Community Care Options $21,600.00 $21,600.00 57 - Lyons, Riverside Townships COMMUNITY CARE OPTIONS Case Management FCS services include intensive outreach and case management, crisis intervention, case consultation with other providers; pyschiatric evaluations and medication monitoring, peer mentoring and advocacy through family resource developers.

School age children between the ages of 5-13 years who are exhibiting social, emotional and/or behavioral problems that negatively impact on their school performance and/or attendance, along with their parents are linked to other services as needs are identified, including parent training and substance abuse treatment.
