Referral Process

  

Local Area Networks (LANs) were created to ensure that children and their families have access to the services and supports they need to succeed.  This can only occur through partnerships between families, teachers, mental health providers, social service agencies, juvenile justice personnel, child welfare workers and anyone else that may be able to help the child or family.

 

Cases are referred to the LAN to be reviewed by the Community Team.  If the team accepts the referral, it is recommended that a Child and Family Team be formed.  This team shares responsibility, expertise, and mutual support in designing creative services to meet the child and family’s needs.  The individualized plan is developed by the people who know the child best and can be modified at anytime to meet the changing needs of the child/family.
