LAN 33: Wraparound Referral Process
To make a referral you will need to complete a "LAN Referral Information form". This form is available through the Statewide LAN Website (click on Wraparound Forms at the bottom of this page) or you may request a copy from:

Complete the referral and send or Fax it to the LAN #33 Intake and Referral Committee in care of Betsy Steele (see above). A member of the subcommittee may need to contact you to obtain further information prior to their making a decision to approve or reject the referral. In addition to making a decision regarding eligibility this committee may be able to assist by providing community resource information.
Please note: The referring agent is normally expected to be the facilitator for the Wraparound team. If the referring agent has not been trained in Wraparound they need to inform Betsy Steele at Family Service Agency of DeKalb at the time of the referral.
