LAN 33: Responsibilities of the FCS Review Committee


The primary responsibility of the FCS Review Committee is to review the quarterly reports of the funded agencies and to help assure compliance with the grant requirements.

Each funded agency is required to submit quarterly documentation prior to the authorization of funds.  The narrative portion of the quarterly report is to be mailed or faxed to Jane Addams Community Mental Health Center by the first business day of the month following the end of the quarter, the first of January, April, July, and October.  The remaining statistical and budgetary reports must arrive at Jane Addams by the 10th of the month following the end of the quarter.  Jane Addams staff will forward copies of the compiled narrative and statistical information to the chairman of the Review Committee and copies of the budgetary information to the Regional Office of Education by the 14th of January, April, July, and October.

The Review Committee convenes a meeting four times per year to review the reports that have been submitted.  If an agency fails to submit the required paperwork and the information is not available for review, the committee can not recommend that the funds be authorized for that quarter.  Later paperwork will be reviewed at the next scheduled quarterly review meeting where funding from the preceding quarter can be authorized.

The Review Committee does not monitor the quality of the services provided or do cross checks to verify the written material that is submitted.  The committee assumes all the information provided is accurate and reflective of the good work being provided in this community.

In the event the committee has a concern regarding the content of the reports or believes the grant requirements are not being adhered to, it is the committee’s responsibility to communicate their concern directly to the funded agency.  If the concern pertains to several agencies, the issue will be presented to the steering committee and a suggested solution made to help address the concern. Any recommendation made by the Review Committee will be made by way of a motion to the Steering Committee.

The committee will be composed of four individuals, who can not be employed by or serve on the Board of Directors of a funded agency.  The committee shall elect a chairperson who will serve a two-year term.  In June of each odd calendar year the committee shall elect a new chairperson.  The responsibility of the chairperson include convening the quarterly meetings and submitting minutes of their meetings to the Steering Committee and to each of the funded agencies.
