Wrap About LAN's Wrap Services
Coordinating Agencies:
What
is a LAN? LANs are Local Area Networks. They are made up of various agencies serving JoDaviess and
Stephenson counties. These agencies
get together to develop, coordinate, and help deliver supportive social
services. What
is the Wraparound Staffing Process? To help create services on a “one kid at a
time” basis to support options for students with
complex needs and their families, all agencies involved and the family
get together to agree to an overall plan. Who
is Eligible for Wraparound? Referrals are made from agencies, area school
districts, probation departments, DCFS, and families who are looking for
assistance. The process is targeted
for children and youth at risk. How
Do I Access Wrap Services? The referral source will contact the Youth Service
Coordinator at (815) 599-4300, or if the child is a Department of Children and
Family Services (DCFS) ward, the targeted case manager at the Freeport DCFS
office at 235-7878, and request a Wraparound Staffing. What
is a Child and Family Team? Members from the family, social service agencies,
and other community representatives that work together to design and implement
plans to support at risk youth. What
Needs Do Wrap Plans Address? The child and family team builds supports for the
whole family in various areas of their life:

is the Child and Family Team Put Together?
Where
Do the Teams Meet for Wrap Planning? Often this takes place in a participating agency.
The family may request other locations as well. What
Are the Steps for Individualized Planning?
What
Funding is Available to the Child and Family Team? All other funding sources will be explored and used
for the child and family. Teams may
apply for limited flex funds controlled by the LAN Steering Committee.
Funds are for supplementing gaps in service needs and supports for the
child and family in reaching planning team goals.
